I understand that I will be participating in physical movements, opportunities for relaxation, neuromuscular re-education, and\/or stress relief. As with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. If I experience any pain or discomfort, I will listen to my body, adjust the posture, and ask for support from the instructor.<\/p>

Exercise is not a substitute for medical attention, examination, diagnosis or treatment. Exercise is not recommended and is not safe under certain medical conditions. I affirm that I alone am responsible to decide whether to participate in exercise. I, on behalf of myself, my heirs, personal representatives, do hereby irrevocably release, waive, discharge and covenant not to sue claims arising from the negligence of The Peacemaker, its affiliates, its teachers or instructors, its owners and operators, from liability for any and all claims arising from any of the aforementioned activities.<\/span><\/p>\n

This agreement applies to 1) personal injury from accidents or illnesses arising from the observation and\/or participation in activities on premises, and individual use of facilities or equipment; and to 2) any and all claims resulting from damage, loss, or theft of property.<\/span><\/p>\n

I\u2019ve read the previous paragraphs and I know the nature of this activity. I understand the demands such activities present for my physical condition and my skill level. I hereby assert that my participation is voluntary and that I knowingly assume all such risks. I expressly agree that this waiver and assumption of risks agreement is intended to be as broad and inclusive as permitted by the laws of the State of Florida and intend for my signature to be a complete and unconditional release of all liability to the greatest extent allowed by this state.<\/span><\/p>\n

I also release ownership and authorize the use of any likeness and image, either in print media format or video, in perpetuity for the promotion of The Peacemaker.
